Anyone enrolling for classes in 2012 is required to enrol either via the form below or in person at our Studio.

Unit 2, 30 Barry Road, Chipping Norton
You can come down to our Studio to enrol during our opening hours. You will need to complete an enrolment form and pay the current term's fees in advance plus $25.00 registration fee (registration fee will not apply if you are on a Silver or Gold Membership). If you enrol via this website an invoice will be emailed to you with our bank account details. No enrolments will be accepted without all of the above.

By submitting this enrolment form you are acknowledging the following. You are aware there is an inherent risk of injury or ill health resulting from the facilities and services from the participation in dance programs generally.
You therefore undertake to utilize these services at your own risk and hereby waive on behalf of yourself, your heirs and executors hereafter against the Glenda Benton Academy of Dancing Pty Ltd for any injury, illness or adverse changes in your medical condition or state of health (whether permanent or temporary), arising directly or indirectly from your use of the services provided, whether supervised or not by the Glenda Benton Academy of Dancing Pty Ltd staff.
You understand that you agree to pay all fees one term in advance.
You authorise us in the event of any medical emergency to obtain all necessary medical assistance and treatment. You agree to pay all expenses on behalf of your child/ren.
